Chemotherapy-Induced Nausea & Vomiting

IV emulsion (aprepitant - Cinvanti)

  • Indicated in combination with other antiemetic agents for prevention of
    • Acute and delayed nausea and vomiting associated with initial and repeat courses of highly emetogenic cancer chemotherapy (HEC) including high-dose cisplatin as a single-dose regimen
    • Delayed nausea and vomiting associated with initial and repeat courses of moderately emetogenic cancer chemotherapy (MEC) as a single-dose regimen
    • Nausea and vomiting associated with initial and repeat courses of MEC as a 3-day regimen
  • HEC
    • Day 1: 130 mg IV infused over 30 minutes or as IV push over 2 min; complete infusion or injection ~30 minutes before chemotherapy plus dexamethasone 12 mg PO and a 5-HT3 antagonist
    • Day 2: Dexamethasone 8 mg PO in AM
    • Days 3 and 4: Dexamethasone 8 mg PO BID (morning and evening)
    • Note: A 50% dosage reduction of dexamethasone on Days 3 and 4 is recommended to account for a drug interaction with aprepitant
  • MEC (single-dose regimen)
    • Day 1: 130 mg IV infused over 30 minutes or as IV push over 2 min; complete infusion or injection ~30 minutes before chemotherapy plus dexamethasone 12 mg PO and a 5-HT3 antagonist
    • Note: A 50% dosage reduction of dexamethasone is recommended to account for a drug interaction with aprepitant
  • MEC (3-day regimen plus aprepitant PO)
    • Day 1: 100 mg IV infused over 30 minutes or as IV push over 2 min; complete infusion or injection ~30 minutes before chemotherapy plus dexamethasone 12 mg PO and a 5-HT3 antagonist
    • Days 2-3: 80 mg PO 1 hr prior to chemotherapy

Postoperative Nausea & Vomiting

IV emulsion (aprepitant – Aponvie)

  • Indicated for prevention of postoperative nausea and vomiting (PONV) in adults
  • 32 mg IV administered as a 30-second injection before induction of anesthesia

Dosage Modifications

Renal impairment

  • Severe (CrCl <30 mL/min): Decreased AUC of total aprepitant (unbound and protein bound) by 21% and Cmax decreased by 32%, compared to healthy subjects (CrCl >80 mL/min)
  • ESRD undergoing hemodialysis: Decreased AUC of total aprepitant by 42% and Cmax decreased by 32%

Hepatic impairment

  • Mild-to-moderate (Child Pugh score 5 to 9): No dosage adjustment necessary
  • Severe (Child Pugh score >9): Safety and efficacy has not been established

            • apalutamide

              apalutamide will decrease the level or effect of aprepitant by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Coadministration of apalutamide, a strong CYP3A4 inducer, with drugs that are CYP3A4 substrates can result in lower exposure to these medications. Avoid or substitute another drug for these medications when possible. Evaluate for loss of therapeutic effect if medication must be coadministered. Adjust dose according to prescribing information if needed.

            • avapritinib

              aprepitant will increase the level or effect of avapritinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Avoid coadministration of avapritinib with moderate CYP3A4 inhibitors. If unable to avoid, reduce avapritinib starting dose. See drug monograph Dosage Modifications.

After discontinuation of a moderate CYP3A inhibitor, resume cobimetinib 60 mg. Use an alternative to a moderate CYP3A inhibitor in patients who are taking a reduced dose of cobimetinib (40 or 20 mg daily).

            • colchicine

Avoid use of colchicine with strong CYP3A4 inhibitors. If coadministration is necessary, decrease colchicine dose or frequency as recommended in prescribing information. Moderate CYP3A4 inhibitors are not recommended with eliglustat poor or intermediate metabolizers; reduce eliglustat dose from 84 mg BID to 84 mg once daily in extensive metabolizers .

            • encorafenib

              aprepitant will increase the level or effect of encorafenib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If concomitant use of a moderate CYP3A4 inhibitor is unavoidable, reduce encorafenib dose to one-half of the dose (eg, reduce from 450 mg/day to 225 mg/day). After discontinuing the inhibitor for 3-5 elimination half-lives, resume previous encorafenib dose.

            • entrectinib

              aprepitant will increase the level or effect of entrectinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Avoid coadministration of moderate CYP3A4 inhibitors with entrectinib, a CYP3A4 substrate. If coadministration unavoidable, reduce dose to 200 mg/day for patients aged 12 y or older with BSA >1.50m2. Resume previous entrectinib dose after discontinuing moderate CYP3A inhibitor for 3-5 elimination half-lives.

            • enzalutamide

              enzalutamide will decrease the level or effect of aprepitant by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • ergotamine

              aprepitant will increase the level or effect of ergotamine by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Contraindicated.

            • erythromycin base

Avoid coadministration of fedratinib (a CYP3A4 and CYP2C19 substrate) with dual CYP3A4 and CYP2C19 inhibitor. Effect of coadministration of a dual CYP3A4 and CYP2C19 inhibitor with fedratinib has not been studied.

            • fentanyl

              aprepitant will increase the level or effect of fentanyl by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.             • lovastatin

              aprepitant will increase the level or effect of lovastatin by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Contraindicated.

            • lurbinectedin

              aprepitant will increase the level or effect of lurbinectedin by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • midazolam intranasal

              aprepitant will increase the level or effect of midazolam intranasal by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Coadministration of moderate CYP3A4 inhibitors with midazolam intranasal causes higher midazolam systemic exposure, which may prolong sedation.

            • mifepristone

              mifepristone will increase the level or effect of aprepitant by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • mobocertinib

              aprepitant will increase the level or effect of mobocertinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If use of moderate CYP3A4 inhibitor unavoidable, reduce mobocertinib dose by ~50% (eg, 160 to 80 mg); closely monitor QTc interval.

            • naloxegol

              aprepitant will increase the level or effect of naloxegol by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If coadministation of naloxegol with moderate CYP3A4 inhibitors is unavoidable, reduce naloxegol dose to 12.5 mg qDay

            • nefazodone

              nefazodone will increase the level or effect of aprepitant by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • neratinib

              aprepitant will increase the level or effect of neratinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Avoid coadministration of neratinib with strong/moderate CYP3A4 inhibitors.

            • olaparib

              aprepitant will increase the level or effect of olaparib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If coadministration with moderate CYP3A inhibitors cannot be avoided, reduce olaparib dose to 200 mg (capsule) or 150 mg (tablet) PO BID. Do not substitute tablets with capsules.

            • omaveloxolone

              aprepitant will increase the level or effect of omaveloxolone by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If unavoidable, reduce omaveloxolone dose to 100 mg/day. Closely monitor for adverse effects. If adverse effects emerge, further reduce to 50 mg/day.

            • pacritinib

              aprepitant will decrease the level or effect of pacritinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • pazopanib

              aprepitant will increase the level or effect of pazopanib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Avoid coadministration of pazopanib with strong CYP3A4 inhibitors if possible; if must coadminister, decrease pazopanib dose to 400 mg/day

            • pemigatinib

              aprepitant will increase the level or effect of pemigatinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If coadministration with strong or moderate CYP3A4 inhibitors is unavoidable, reduce pemigatinib dose (refer to drug monograph dosage modifications). After discontinuing the CYP3A4 inhibitor for 3 elimination half-lives, may resume previous pemigatinib dose.

            • pexidartinib

              aprepitant will increase the level or effect of pexidartinib by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. If coadministration with strong or moderate CYP3A4 inhibitors is unavoidable, reduce pexidartinib dose (refer to drug monograph dosage modifications). After discontinuing the CYP3A4 inhibitor for 3 elimination half-lives, may resume previous pexidartinib dose.

            • pimozide

              aprepitant will increase the level or effect of pimozide by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Contraindicated. Risk of QT interval prolongation.

            • ranolazine

            • rifabutin

            • rifampin

              rifampin will decrease the level or effect of aprepitant by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • ritonavir

              ritonavir will increase the level or effect of aprepitant by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

            • saquinavir

            • selumetinib

            APREPITANT - ORAL

            COMMON BRAND NAME(S): Emend

            USES: Aprepitant is used with other medications to help prevent nausea and vomiting caused by cancer drug treatment (chemotherapy). Aprepitant works by blocking one of the body's natural substances (substance P/neurokinin 1) that causes vomiting.This medication will not treat nausea or vomiting that has already started. Ask your doctor what you should do if you already have nausea or vomiting.

            HOW TO USE: Read the Patient Information Leaflet if available from your pharmacist before you start taking aprepitant and each time you get a refill. If you are taking the liquid form of this medication, read the Instructions for Use sheet for directions on the proper use of this medication. If you have any questions, ask your doctor or pharmacist.Take this medication by mouth with or without food as directed by your doctor. If you are taking capsules, swallow the capsules whole.Take the first dose as directed by your doctor, usually 1 hour before the start of chemotherapy. For the next 2 days, continue to take a dose once daily 1 hour before the start of treatment if you are getting chemotherapy that day. If you are not getting any chemotherapy, then take the dose once daily in the morning.The dosage is based on your medical condition, age, and response to treatment. In children, the dosage is also based on weight. It is important to follow your doctor's instructions for when to take each dose, how long to keep taking it, and the strength (number of milligrams) of each dose. If you have any questions, ask your doctor or pharmacist.Tell your doctor if you vomit or feel nauseated.

            SIDE EFFECTS: Tiredness or hiccups may occur. If either of these effects lasts or gets worse, tell your doctor or pharmacist promptly.Remember that this medication has been prescribed because your doctor has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.This is not a complete list of possible side effects.             DRUG INTERACTIONS: Drug interactions may change how your medications work or increase your risk for serious side effects. This document does not contain all possible drug interactions. Keep a list of all the products you use (including prescription/nonprescription drugs and herbal products) and share it with your doctor and pharmacist. Do not start, stop, or change the dosage of any medicines without your doctor's approval.Other medications can affect the removal of aprepitant from your body, which may affect how aprepitant works. Examples include azole antifungals (such as itraconazole, ketoconazole), diltiazem, macrolide antibiotics (such as clarithromycin, erythromycin), nefazodone, HIV protease inhibitors (such as nelfinavir), rifamycins (such as rifampin, rifabutin), ritonavir, St. John's wort, certain anti-seizure medicines (such as carbamazepine, phenytoin), among others.Aprepitant can both speed up or slow down the removal of other medications from your body, which may affect how they work. Examples of affected drugs include elacestrant, flibanserin, lomitapide, pimozide, among others.If you take warfarin, this drug may affect how well warfarin works in your body. Your doctor should test your blood during the 2 weeks after your aprepitant treatment to measure how well warfarin is working.This medication may decrease the effectiveness of hormonal birth control such as pills, patch, or ring. This could cause pregnancy. Discuss with your doctor or pharmacist if you should use additional reliable birth control methods while using this medication and for 1 month after stopping this medication. Also tell your doctor if you have any new spotting or breakthrough bleeding, because these may be signs that your birth control is not working well.Aprepitant is very similar to fosaprepitant. Do not use fosaprepitant while using aprepitant.

            OVERDOSE: If someone has overdosed and has serious symptoms such as passing out or trouble breathing, call 911. Otherwise, call a poison control center right away. US residents can call their local poison control center at 1-800-222-1222. Canada residents can call a provincial poison control center.

            NOTES: Do not share this medication with others.

            MISSED DOSE: It is important to take each dose of this medication as scheduled. If you miss a dose, ask your doctor or pharmacist right away for a new dosing schedule. Do not double the dose to catch up.

            STORAGE: Store the capsules at room temperature away from light and moisture. Store the liquid in the refrigerator and discard any unused liquid after 72 hours. Do not freeze. When ready to use, the liquid form of this medication may be stored at room temperature for up to 3 hours. Do not store in the bathroom. Keep all medications away from children and pets.Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed. Consult your pharmacist or local waste disposal company.
