            Coadministration with strong CYP3A inducers

            Cautions

            Residual concentrations of lenacapavir may remain in systemic circulation for prolonged periods (≥12 months after last SC dose); counsel patients that maintenance dosing by injection is required every 6 months, because missed doses or nonadherence could lead to loss of virologic response and development of resistance

            Local injection site reaction may occur, manifested by swelling, pain, erythema, nodule, induration, pruritus, extravasation, or mass

            Immune reconstitution syndrome

            • Immune reconstitution syndrome reported; during initial phase of combination antiretroviral therapy (ART), patients whose immune system responds may develop an inflammatory response to indolent or residual opportunistic infections (eg, MAC, CMV, PCP, TB), which will likely require treatment

            The CDC recommends that HIV-1-infected mothers in the United States not breastfeed their infants to avoid risking postnatal transmission of HIV-1 infection

            Mechanism of Action

            HIV-1 capsid inhibitor; HIV capsid is, a protein shell that protects HIV’s genetic material and enzymes needed for replication

            Lenacapavir directly binds to the interface between capsid protein (p24) subunits in hexamers

            Capsid binding interferes with multiple essential steps of the viral lifecycle, including

            • Capsid-mediated nuclear uptake of HIV-1 proviral DNA (by blocking nuclear import proteins binding to capsid)
            • Virus assembly and release (by interfering with Gag/Gag-Pol functioning, reducing production of capsid protein subunits)
            • Capsid core formation (by disrupting the rate of capsid subunit association, leading to malformed capsids)

            SC Preparation

            Injection kit components are for single use only; use of a vial access device is required

            Use aseptic technique; visually inspect solution in vials and prepared syringe for particulate matter and discoloration before administering

            Solution should appear yellow; discard if solution is discolored or contains particulate matter

            Once solution is withdrawn from vials, use SC injections as soon as possible

            Attach needle to syringe, expel any air bubbles in syringe and prime to 1.5 mL
