Early mechanical intervention (primary PCI) or pharmacologic reperfusion should be performed as soon as possible for patients with clinical presentation of STEMI within 12 hours of symptom onset and who have persistent ST-segment elevation. In addition, it is reasonable to consider an early reperfusion strategy for patients presenting after more than 12 hours, provided that there is clinical and/or ECG evidence of ongoing ischemia, with primary PCI being the preferred method in this population.
