Although not recommended outright, metabolic and bariatric surgery should be considered in certain patients with type 2 diabetes mellitus and class I obesity who have poorly controlled hyperglycemia despite optimal medical treatment. Regardless of whether the optimal medical treatment is oral or injectable (including insulin), metabolic and bariatric surgery may be considered in these patients.
