Editor's Note:
The Case Challenge series includes difficult-to-diagnose conditions, some of which are not frequently encountered by most clinicians but are nonetheless important to accurately recognize. Test your diagnostic and treatment skills using the following patient scenario and corresponding questions. If you have a case that you would like to suggest for a future Case Challenge, please contact us.
Background
A 23-year-old Pakistani man presents to an outpatient department with a 3-year history of bilateral progressive hearing loss, tinnitus, and vertigo. At the time of presentation, he also describes a diffuse headache, blurring of vision, and swaying from side to side upon walking over the past 4 months.
The patient has no history of limb weakness, sensory symptoms, or seizures. He is single and unemployed. He is a nonsmoker and denies substance abuse. He reports that his mother died after being operated on for a brain neoplasm. However, he does not have any relevant documents with him.
