Those who undergo small-bowel transplantation have a high risk for developing GVHD. The incidence of GVHD increases with unrelated matched donor transplants compared with related matched transplants.

On liver function tests (eg, bilirubin, aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, total protein, albumin), elevation of the alkaline phosphatase concentration is an early sign of liver involvement by GVHD.

A phase 3 clinical trial showed that the use of MMF does not significantly improve the incidence of chronic GVHD at 1 year. Additionally, MMF is associated with leukopenia.
