CLL and MCL are both considered cancers of the small to medium-sized lymphocytes and have some similarities by immunohistochemistry and presentation. A significant difference is that expression of CD20 is dim in CLL, whereas it is bright in MCL. Moreover, MCL typically expresses FMC-7, as well as CD5 and CD19 antigen, but not CD23. Clinically, the two diseases may have a similar presentation; however, MCL is more aggressive than CLL.
