The imaging modality of choice for patients with suspected IPF is HRCT. After a multidisciplinary discussion, patients with a radiological pattern of UIP in the appropriate clinical setting (e.g. 60 years old, male, smoker) can receive a diagnosis of IPF. Nuclear imaging is not currently a part of the diagnostic algorithm of IPF.
In patients with IPF, perfusion lung scintigraphy may show nonspecific, subsegmental mismatched perfusion defects. These findings, however, do not correlate with clinical disease severity.
Nuclear imaging may be useful in patients with IPF, but Gallium-67 imaging has not proven to be of value in patients with established IPF.
