Chronic pain and stiffness are the most common complaints of patients with AS. Symptoms include those related to inflammatory back pain, peripheral enthesitis and arthritis, and constitutional and organ-specific extra-articular manifestations.

Because AS is a systemic inflammatory disease, systemic features are common. Fatigue is a common complaint, occurring in approximately 65% of patients with AS. Most patients report moderately severe fatigue. Fever and weight loss may also occur during periods of active disease.
