According to 2019 guidelines from the ACR/Arthritis Foundation, strong recommendations include the following:

  • Exercise

  • Weight loss in patients with knee and/or hip osteoarthritis who are overweight or obese

  • Self-efficacy and self-management programs

  • Tai chi

  • Cane use

  • Hand orthoses for first CMC joint osteoarthritis

  • Tibiofemoral bracing for tibiofemoral knee osteoarthritis

  • Topical NSAIDs for knee osteoarthritis

  • Oral NSAIDs

TENS is strongly recommended against in patients with knee and/or hip osteoarthritis. Hydroxychloroquine is strongly recommended against in patients with knee, hip, and/or hand osteoarthritis. Iontophoresis is conditionally recommended against in patients with first CMC joint osteoarthritis.
