According to one meta-analysis, the relative risk for hip fracture is 2.6 for each SD in hip BMD obtained by DXA. In addition, BMD assessments by DXA to predict hip fracture are more predictive when measurements are obtained at the hip rather than at the spine or forearm.

According to guidelines from the International Osteoporosis Foundation (IOF) and European Society for Clinical and Economic Aspects of Osteoporosis (ESCEO) and Osteoarthritis, several factors contribute significantly to fracture risk beyond that provided by BMD measurements. These include age, sex, low body mass index, and a parental history of hip fracture, among others.

Measurement of BMD at the femoral neck can help to predict hip fracture risk in men and women. The NHANES III reference database for femoral neck measurements in women aged 20-29 years is recommended by the IOF, ESCEO, the World Health Organization, the National Osteoporosis Foundation (NOF), and the International Society for Clinical Densitometry for the diagnosis and assessment of osteoporosis. Using these criteria, the risk for hip fracture for any age and BMD at the femoral neck is approximately the same in men and women.
