Early evaluation for lung transplant should be considered in patients with advanced disease that is refractory to medical therapy. Patients with IPF have a shorter median time on lung transplant waiting lists than patients with other diagnoses; however, they have a higher mortality rate.

Several studies have described improved survival with double-lung transplant compared with single-lung transplant in pulmonary fibrosis. However, a recent study examining recipients concurrently listed for single-lung transplant and double-lung transplant found that overall survival was similar regardless of procedure.
