Alprazolam withdrawal syndrome is generally considered more severe than other benzodiazepine withdrawal syndromes, even when tapered according to manufacturer guidelines. Alprazolam and alprazolam-XR are considered to have the same general withdrawal risk as other benzodiazepines. However, alprazolam withdrawal syndrome is more commonly associated with delirium and psychosis compared with withdrawal from other benzodiazepines.
The use of other benzodiazepines in patients with alprazolam withdrawal syndrome has been reported, but it is still under investigation. It should be noted that carbamazepine and clonidine have shown benefit in patients with alprazolam withdrawal syndrome. Carbamazepine may help improve sleep disturbance, anxiety, and mood instability, which are commonly associated with withdrawal.
