The following are among the recommendations from the ACG released in January 2021:

  • Serologic testing to rule out celiac disease is routinely recommended in patients with IBS and diarrhea symptoms.

  • Testing for food allergies and food sensitivities is not recommended in patients with IBS unless reproducible symptoms concerning for a food allergy are noted.

  • Routine colonoscopy is not recommended in patients with IBS symptoms who are younger than 45 years without warning signs.

  • The ACG recommends against routine stool testing for enteric pathogens in all patients with IBS.
