The American Gastroenterological Association recommends bidirectional endoscopy to evaluate asymptomatic postmenopausal women and men with iron deficiency anemia.
Conversely, women who seek to avoid the risks inherent in endoscopy and are not concerned about missing gastrointestinal cancer may choose initial iron therapy over bidirectional endoscopy, especially young patients whose history suggests other etiologies of iron deficiency anemia.
However, this recommendation does not apply to patients with gastrointestinal symptoms. In general, the workup will vary according to an individual patient's underlying risk of serious gastrointestinal diseases and in men and women of different ages.
