The stages of diabetic nephropathy, as measured by GFR (mL/min/1.73 m2), are:

  • Stage 1: normal or slightly elevated: GFR ≥ 90

  • Stage 2: mild: GFR 60-89

  • Stage 3: moderate: GFR 30-59

  • Stage 4: severe: GFR 15-29

  • Stage 5: end-stage: GFR < 15

Stage 3 may be separated into two stages: stage 3a, GFR 45-59; and stage 3b, GFR 30-44. In both stages, the patient is monitored closely and treated for worsening hypertension.
