Patients with ILD commonly exhibit abnormal sleep architecture and increased sleep fragmentation on polysomnography. Fatigue is a frequent complaint, and it is likely that poor sleep quality is a significant contributor. A number of studies have shown that sleep-disordered breathing is prevalent in this population, particularly in the IPF subgroup.

Inherited ILDs may be divided into two major categories: systemic disorders affecting multiple organs and disorders that primarily affect the lung. For both groups, thorough past medical and family histories provide essential clues about the exact nature of the ILD.

Continuous exposure to environmental antigens has been linked to the development and progression of hypersensitivity pneumonitis; feathers, molds, organic dust, moldy hay, and synthetic compounds such as isocyanates are the most common offending antigens.

ILD is the most common indication for lung transplantation, and transplantation has been shown to prolong survival and improve symptoms in PF-ILD.
