According to cholesterol management guidelines from the American Heart Association/American College of Cardiology, high-intensity statin therapy (or the maximally tolerated statin) is recommended for patients with ASCVD. Greater reductions in LDL-C levels equate greater reductions in cardiovascular risk. Other guideline recommendations include:
In patients 40-75 years of age with diabetes and LDL-C ≥ 70 mg/dL (≥ 1.8 mmol/L), a moderate-intensity statin should be prescribed without calculating 10-year ASCVD risk
Moderate-intensity statins are recommended for patients with a ≥ 7.5% 10-year risk for ASCVD
In very high-risk ASCVD (a history of multiple major ASCVD events or one major ASCVD event and multiple high-risk conditions), an LDL-C threshold of 70 mg/dL (1.8 mmol/L) should be used to consider addition of nonstatins to statin therapy
