Most anal fissures occur in the posterior midline, with the remainder mostly occurring in the anterior midline. Patients rarely present with both anterior and posterior fissures. Fissures occurring off the midline (laterally) should raise the possibility of other bowel conditions (eg, Crohn's disease), infection (eg, sexually transmitted disease or AIDS), or cancer.
