Patients diagnosed with NPD are more likely to have comorbid axis I diagnoses, such as anxiety disorders, bipolar disorder, major depressive disorder, substance-related disorders (particularly related to alcohol and cocaine), and anorexia nervosa. NPD has a higher risk for death by suicide compared with other personality disorders.

NPD is more common among men than women. NPD manifests by the early to mid-20s; however, as physical attractiveness declines or infirmities begin, it may worsen in middle or old age.
