The ACR recommends a target uric acid level < 6.0 mg/dL for all patients with gout. For patients with tophaceous gout, a uric acid < 5.0 mg/dL is recommended. Observational studies suggest that patients treated with urate-lowering therapy who achieved serum uric acid levels < 6 mg/dL have a 37% reduction in renal disease progression. In addition, maintaining a serum uric acid < 6 mg/dL may reduce the number or size of tophi, pain, and frequency of attacks.
