For patients with blast phase CML, induction chemotherapy in addition to a TKI is recommended, with the type of induction chemotherapy being guided by the myeloid or lymphoid lineage of the blasts. If a patient is not a candidate for induction chemotherapy, NCCN guidelines recommend a TKI; steroids may be added for patients with lymphoid blasts. A clinical trial may also be considered.
