Guidelines for the management of patients with intracranial atherosclerotic disease (ICAD) were published in September 2022 by the European Stroke Organisation (ESO) in European Stroke Journal.[1]
Oral anticoagulation is not recommended over aspirin in patients with an ischemic stroke or transient ischemic attack due to high-grade stenosis related to intracranial atherosclerotic disease (ICAD).
Angioplasty and/or stenting added to best medical treatment as first-line treatment is not recommended in patients with an ischemic stroke or transient ischemic attack related to a high-grade stenosis due to ICAD.
Neurosurgical procedures are not recommended in patients with an ischemic stroke or transient ischemic attack related to a high-grade stenosis due to ICAD.
It is suggested to consider ischemic pre-conditioning as an adjuvant to best medical treatment in patients with an ischemic stroke or transient ischemic attack related to a high-grade stenosis due to ICAD.
Aggressive vascular risk factor control, including lipid management and lifestyle changes (ie, increased physical activity) is recommended in patients with an ischemic stroke or transient ischemic attack related to an intracranial atherostenosis to improve outcomes.
