Indications for treatment include pernicious anemia or vitamin B12 deficiency due to dietary deficiencies, folic acid deficiency, parasitic infestation, inadequate intrinsic factor, or inadequate utilization of B12 are among the FDA-approved indications for cobalamin. Some drugs (eg, metformin) may impair vitamin B12 absorption by interfering with vitamin B12–intrinsic factor, resulting in B12 deficiency.

Non-FDA approved indications include cyanide poisoning, folic acid deficiency, smoke inhalation, and surgery-associated vasoplegia.
