In patients with Leber optic nerve atrophy, cyanocobalamin should be used cautiously because it can increase disease severity. Prudent use of cyanocobalamin is also recommended in renal failure because of the presence of an aluminum component. Cyanocobalamin has not been established to worsen disease severity of macular degeneration, major depressive disorder, and vestibular neuronitis.
