The mainstay of treatment for patients with attention-deficit/hyperactivity disorder (ADHD) remains pharmacologic therapy, particularly stimulants, which are effective in about 70% of patients. These may be methylphenidates or amphetamines, which block the reuptake of dopamine at the presynaptic and postsynaptic membranes. (Amphetamines also release dopamine directly.) Nonstimulant pharmacologic therapy, such as antidepressants and alpha agonists, are also in demand; however, many patients and families are concerned about adverse effects of stimulants, such as decreased appetite and sleep, changes in blood pressure, and risk for dependency.
